Objective: to describe the construction stages and validation evidence of the instrument Quality and Patient Safety from the Perspective of Primary Health Care Users. Methods: Methodological research developed in six stages: categorization of the main patient safety incidents in Primary Health Care; instrument construction; content validation; semantic validation; content revalidation and factor analysis. Results: the questionnaire obtained a content validity index (0.869), intraclass correlation coefficient (0.982) and Cronbach’s alpha (0.985) in two rounds of validation by experts with 35 items. The exploratory factor analysis resulted in 26 items distributed into six factors, with Kaiser-Meyer-Olkin= 0.740 and the sphericity test of Bartlett p <0.001. Conclusion: the instrument, both due to its factorability and internal consistency, is suitable for evaluating patient safety from the perspective of the Primary Care user, enabling the study of risk control and mitigation measures, in order to provide greater patient engagement.
